Navigator
As a navigator, you’ll be at the heart of our mission of improving access to pediatric care for vulnerable children. On our bus traveling all over the northern part of Montreal, you’ll support young children and their families by helping them navigate the healthcare system.
Principle responsibilities
- Conduct psychosocial assessments and outreach work with marginalized children aged 0 to 5 and their families.
- Act as a point of contact for families and take on the role of the case worker within our team.
- Facilitate connection and dialogue between families and the healthcare network, in a context of cultural diversity.
- Help families navigate the healthcare network and encourage them to become autonomous.
- Create and adapt tools and educational materials for families.
- Help the project grow by developing and maintaining real connections between the healthcare ecosystem and community organizations.
Qualifications
- A diploma in social work or a related field.
- Relevant experience in the community sector working with families and vulnerable populations, immigrants in particular.
- Excellent interpersonal and communication skills to establish trusting relationships with families.
- Strong team spirit and talent for working in a multidisciplinary team.
- Faire preuve d’une excellente capacité d’adaptation et d’organisation.
- Fluency in French or English, and a second language an asset.
- Be comfortable working in a confined space (the mobile clinic is set up in a bus).
