Nurse practitioner specialized in primary care
As a nurse practitioner specialized in primary care at DocTocToc, you’ll be at the heart of our mission of improving access to pediatric care for vulnerable children. On our bus traveling all over Montreal North, you’ll perform clinical assessments of children aged 0 to 5 while considering the social determinants of health. You’ll work in collaboration with our nurse clinicians to assess and follow-up with patients.
Principal responsibilities
- Perform clinical assessments of children aged 0 to 5 in situations of vulnerability and providing local care services.
- Assist in triage of consultation requests from community partners.
- Promote health and disease prevention among families while encouraging their active participation in care.
- Collaborate with our interdisciplinary team of psychosocial workers, nurse clinicians and pediatricians.
- Assist in the coordination of clinical activities with our interdisciplinary team and external partners.
- Participate in creating, implementing and updating clinical processes while collecting data to evaluate their impacts and performance.
- Help the project grow by developing and maintaining real connections between the healthcare ecosystem and community organizations.
Qualifications
- Membership with the Ordre des infirmières et infirmiers du Québec (OIIQ) and an SNP permit with no restrictions for the province of Quebec.
- Relevant experience as a nurse practitioner specialized in primary care.
- Excellent interpersonal and communication skills to establish trusting relationships with families.
- Good teamwork skills.
- Strong organizational skills and adaptability.
- Fluency in French or English, and a second language an asset.
- Be comfortable working in a confined space (the mobile clinic is set up in a bus).
